I don’t remember my email / password on my patient portal. I cannot access my medical records.

In some patient portals, you can reset your password or have your user ID sent to you (see example below). Otherwise, contact your health network for instructions to retrieve your user ID and/or password. If you need additional help, please contact us at support@embleema.com, we will be happy to assist!
